Bring in copies of all recent lab work on the day of your visit or fax your completed Record Release Authorization to David Cave at 623-551-1768 prior to your visit, so that we may request copies of your lab work for you.

Download, print and once you've completed the New Patient Intake Form, please fax it to 623-551-1768 a few days prior to your scheduled visit. Doing so allows your doctor time to completely review and be familiar with your medical history before you arrive.

Download, print and complete the Consent to Treat Form. This form explains all procedures offered at East-West Integrated Healthcare that you may need over time. Take time to thoroughly understand the form and ask us questions.

Download and print our Financial Agreement and bring the completed copy of page 3 and 4 to your first visit. We want you to always be fully aware of financial responsibilities, never being left to guess.

Download and print the notice of Privacy Agreement (a government mandated notice of HIPPA) and complete page 5 Privacy Agreement. This form explains your patient privacy rights. Please bring this page to your first office visit.
